Job Title: Patient Services Coordinator - Inpatient Medicine/Surgery
- The Patient Services Coordinator (Social Work) plays a crucial role to assist the Social Worker by performing specific tasks in order to help facilitate throughput and discharge.
- The incumbent serves as liaison for patient care activities for the Social Work department and works to provide and improve continuity to the patient's experience.
- Assists patients/care partners with concrete needs as follows: Transportation / Uncomplicated DME / Medication pickup and delivery from internal/external pharmacies / Parking passes from Security.
- Facilitates tasks related to discharge including, but not limited to, distribution of MetroCard, petty cash and patient clothing from the Social Work central office.
- Works cohesively with Social Worker in obtaining community resources and support services (e.g. Meals on Wheels, food pantries, etc.).
- Collaborates with Social Worker, to resolve issues impacting patients' care progression and will assist with resumption of home care services for MLTC patients.
- Monitors and updates the medical record in accordance with standards set for the Department of Social Work and/or regulatory agencies (when applicable).
- Under guidance of Social Worker, assists with referrals to REAP through Epic.
- Under guidance of social worker, assists with referrals to NYLAG, following the established protocols.
- Provides front-end coverage as needed; performs specific assignments relevant to particular programs.
- Provides recommendation for process/service improvement to promote patients satisfaction and works with Social Worker to address issues.
- Please note that this is a non-patient facing role and does not involve direct contact with patients.
- Related experience in the social service and/or health care field preferred.
