General Summary/Overview Statement Newborn Intensive Care Unit

: The Richard and Susan Smith Family Foundation Newborn Intensive Care Unit (NICU) at Brigham and Women’s Hospital (BWH) is a 66-bed Level III newborn intensive care unit that provides complex multidisciplinary care to more than 3,000 babies each year. Although two-thirds of these babies are evaluated briefly and have short stays, more than 1,000 are admitted each year for newborn intensive care. We are the largest high-risk delivery service in New England chosen by many patients in our region as well as from other areas of the United States and by many international patients. Although most of our NICU babies are born at BWH, an increasing number are transferred to BWH from other hospitals. As an academic medical center, we strive to achieve excellence in newborn care as well as conducting groundbreaking research. Obstetrics: We know that our patients and their families only want the best. Each year, more women choose Brigham and Women’s Hospital for the birth of their baby than any other hospital in Massachusetts. Our experts have led the way in many of the most groundbreaking developments in prenatal care and delivery – changing care for women around the world. With specialized research and services designed for pregnant women and their families, our experts do everything possible to provide the most advanced obstetrical care available. Our comprehensive pregnancy services include: General Obstetrics, Center for Women and Newborns at Mary Horrigan Connors Center for Women’s Health, Maternal Fetal Medicine, Comprehensive Care Center for Multiples, Center for Fetal Medicine and Prenatal Genetics, Midwifery, Newborn Medicine, and Subspecialty Services. Reporting into the OB/NICU Social Work Manager, the Patient Navigator supports the care of our patients in new and innovative ways to increase patient engagement and assist patients in achieving their health care goals. The Patient Navigator improves system throughput and participates in the coordination of a safe and comprehensive discharge plan by supporting and coordinating care for patients within the NICU and post-partum medical floors. The Patient Navigator works closely with the social worker and multidisciplinary teams to identify patients who would benefit from navigator involvement, provides supportive care to these patients and their families, and identifies opportunities for additional supportive services while they await care and/or to facilitate disposition including community resource efforts. The Patient Navigator coordinates the details of appropriate referrals along with the social worker and helps develop referral pathways for efficient access to both internal hospital and external community resources to improve patient throughput. Principal Duties and Responsibilities Meet in person with patients and their families who are OB care, to conduct needs inventories, provide support, referral services Collaborate with multidisciplinary treatment teams regarding patient care and support needs Communicate with and engage outpatient care teams and collateral resources in supportive disposition planning Coordinates and collaborates with a wide range of social agencies, clinics, schools and courts Connect patients who receive their primary care at another institution back to their primary care providers to ensure continuity of care Identify services for which patients and families could engage immediately while awaiting inpatient care and/or services that may help facilitate safe discharge planning Work with OB/NICU Social Work Manager to develop and implement efficient referral pathways to promptly secure care and community resources for patients Flexibly assist with other throughput-related tasks as needed including, but not limited to, transportation, Social Security submissions, housing referrals, etc. Document progress daily in medical record (EPIC) Assist with data collection and tracking as needed Attend department meetings and family team meetings as needed Assist with other department tasks as assigned Qualifications Bachelor’s degree required; concentration in Social Work, Psychology or Health Care preferred. 3-5 years of related work experience desired. Spanish speaking preferred. Experience with care coordination and referrals to community resources. Demonstrated commitment to impacting the care of high-risk patients. Skills/Abilities: Excellent written and verbal communication skills Excellent judgement and organization skills Ability to be flexible and resourceful Assessment and problem-solving skills Familiarity with mental health treatments and services, systems of care and community resources Intermediate proficiency in Microsoft Office: Excel, Teams, and PowerPoint Confidence and ability to work both independently and as part of a multidisciplinary team Qualifications Bachelor’s degree required; concentration in Social Work, Psychology or Health Care preferred. 3-5 years of related work experience desired.
