Summary: The Patient Care Coordinator is responsible for all administrative support to Bridgewell Clinic clients.

Schedule: Monday - Friday, 8:00 AM - 4:00 PM or 9:00 AM - 5:00 PM (40 hours)

Essential Duties & Responsibilities:
  1. Greet and assist visitors and direct all phone calls to the appropriate personnel.
  2. Schedule all intake and follow-up appointments in the Electronic Health Record (EHR) for all prescribers.
  3. Schedule new intakes in the EHR as applicable for salaried clinicians.
  4. Support Intake/Registration Coordinator with all new referrals by ensuring that a copy of the patient's insurance card(s) is scanned into the EHR.
  5. Collect current information such as address, phone number, emergency contacts, insurance coverage, and other key data at the time of scheduled appointments, and ensure updates are entered as necessary in the EHR.
  6. Support optimal functioning of the clinic by sorting and distributing incoming mail, scanning all client documents into the EHR, and ensuring that refills are processed daily. Lab results should be reviewed by the prescribers each day and should be prioritized for scanning.
  7. Maintain updated releases of information, annual "Know Your Rights" forms, and patient and contact information in the EHR, and handled requests for information (subpoenas and client requests) as needed. This may include obtaining records from off-site facilities.
  8. Maintain organized reception, conference room, kitchen, and waiting room areas and ordered office and cleaning supplies as needed. Report any facilities-related issues to the landlord and/or the main office as needed.
  9. Collect co-pays as applicable and record them in the EHR.
  10. Prepare documents for the monthly Utilization Review process.
  11. Obtain, monitor, and renew PT-1s (Prescriptions for Transportation).
  12. Rotate to provide coverage in Bridgewell's other Outpatient Behavioral Health Clinics as needed.
  13. Process inactive/terminated clinic cases, and maintain an annual list of client deaths.
  14. Other duties as assigned by the supervisor
Required Education/Experience:
  1. High School diploma required.
  2. Experience with medical records, confidentiality regulations, and medical office operations.
  3. Formal certification or education in any of these areas is preferred.
  4. Candidate should demonstrate the ability to act as a team player and communicate effectively with a multi-disciplinary team.
  5. Bilingual preferred.
Required Skills/Knowledge:
  1. Candidates must demonstrate excellent written and oral communication skills.
  2. Ability to problem solve effectively and possess excellent organizational skills.
  3. Strong working knowledge and experience with Microsoft Word, and Excel, are required.
  4. Excellent organizational skills and the ability to multi-task
  5. Must be detail-orientated
  6. Excellent customer service skills; Ability to interact with patience and sensitivity in all contact with individuals.
