Role Overview

The Clinical Care Coordinator is responsible for strategizing and prioritizing healthcare for individual patients and broader populations, aiming to foster optimal health outcomes. This role requires a deep understanding of nursing practices and the management of complex patient caseloads, striving to enhance the health of patients, families, and the community through educational initiatives and preventive care in a managed care setting. The coordinator ensures timely access to urgent care, emergency services, inpatient care, and surgical interventions.

  • Evaluates both internal and external referrals to determine patient needs, intensity of care required, insurance coverage, and available resources.
  • Creates comprehensive care plans and collaborates with primary care providers, specialists, and other healthcare professionals to optimize care management strategies and ensure continuity of care.
  • Identifies the educational requirements of patients and their families, developing and implementing tailored teaching methods or making necessary referrals.
  • Facilitates communication and collaboration among patients, families, providers, and payers to coordinate services that enhance access to appropriate care and promote health in a cost-effective manner.
  • Assesses patient care outcomes, interprets data reports, and analyzes trends within patient groups.
  • Records patient information, care plans, interventions, and outcomes in accordance with departmental protocols.

Qualifications

  • A Bachelor's Degree or equivalent in nursing or a healthcare-related field is required. For those starting in this RN role, a BSN must be obtained within seven years.
  • A minimum of two years of relevant clinical nursing experience or case management/managed care experience is required.
  • Five years of relevant clinical nursing or case management/managed care experience is preferred.
  • Current Michigan Registered Nurse (RN) license is mandatory.
  • Certification as a Case Manager (CCM) is preferred upon hire.
