Description and Responsibilities:

Open Sky Community Services, in collaboration with the Department of Corrections (DOC), is excited to announce a first of its kind program in the state of Massachusetts, The After Incarceration Support Program. This program will support the re-entry of returning citizens who have been recently released from the DOC and other individuals who have a history of Justice System Involvement. The Aftercare Incarceration Support Program is community-driven and fosters active community involvement, and engagement. The program offers a safe, judgement free, resource rich environment that breaks down barriers to reintegration while providing access to essential, customized resources, and supports a mission to invest in each individuals success. The scope of services includes outreach and engagement, food and nutrition assistance, educational and job placement services, housing assistance as well as healthcare and addiction treatment.

The Intake Specialist serves as the first point of contact, welcoming individuals and families as they enter the After Incarceration Support Program. They will provide oversight of the front desk by answering phone calls, assisting with staff and space scheduling, manage office mail and deliveries as well as provide and coordinate crisis response supports to members as necessary. The Intake Specialist will also complete intakes and initial needs assessments for individuals, including gathering health insurance information and other eligibility requirements.

Other Key Responsibilities:

  • Assist with data entry for the program including RedCap, EIM/ESM, ehana, the triage tracker and other systems.
  • Evaluate individuals needs and service preferences while simultaneously identifying current resources and services.
  • Complete all required documentation in a timely manner.
Qualifications:
  • High School Diploma, GED, or equivalent, required.
  • Lived experience, or minimum of 1 year experience working with people experiencing homelessness, justice involvement, serious and persistent mental illness, substance use disorder, or other underserved populations required.
  • Valid drivers license and acceptable driving record, required.
