The Community Support Specialist provides integrated, holistic, community-based support services.

ESSENTIAL FUNCTIONS:

Provide services to address activities of daily living, including the management of symptoms associated with psychiatric, physical, and/or substance use disorders per program standards and the 8 Dimensions of Wellness Complete services consistent with the requirements outlined in the expectations worksheet and program manual with the supporting documentation completed in a timely manner (within 72 hours) Maintain all assigned client medical records to program standards Provide referrals, advocacy, information and education to meet individual needs of clients Provide appropriate risk assessments and facilitate access to needed interventions as indicated Provide transportation for clients as needed Monitor client hospitalization and participate in discharge planning and follow per program standards
