GENERAL SUMMARY: The CCS Care Coordinator provides a coordinated and comprehensive array of recovery support services, treatment, and psychosocial rehabilitation services that assist individuals to utilize professional, community, and natural supports to address their needs. The program is person- centered and uses client-directed service plans to describe the individualized services that will support the youth or young adult to achieve their recovery goals. CCS is a community-based program in which the majority of services are provided in clients’ homes and communities. CCS for Youth is a voluntary program that provides support and services youth, young adults and their families, who are coping with mental health or substance abuse diagnosis. ESSENTIAL FUNCTIONS:

Provide care coordination services to each client throughout the duration of enrollment in CCS services. Accepts referrals of eligible consumers and initiate process to assess strengths and needs. Set up teams made of formal and informal resources that make up the support network. Facilitate team meetings to develop a care plan based on strengths, values and preferences that will assist the client to reach favorable and measurable outcomes. Educate consumers on how to utilize community supports, both formal and informal. Assist team in establishing measurable consumer outcomes and help develop plan to reduce any system barriers among community based, Child Welfare or other agencies. Document plans, collect outcome data, prepares all documentation according to program required time frames and maintain weekly timesheets. Attend training and ongoing staff development opportunities to enhance skills. Adheres to St. Charles Youth & Family Services Policies and Procedures. Other duties as assigned. MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

Associates degree in Human Service, or Education required. Bachelors or Master’s preferred 1 year experience working in a mental health related setting Knowledge base of individual and family issues and dynamics. Ability to prepare formal reports. Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors. Strong verbal and written communication skills. Valid Wisconsin driver’s license and liability insurance. Ability to use vehicles for work related duties according to the agency’s insurance liability standards. Ability to accurately perform record keeping and documentation entries. Working knowledge of computer applications preferred. WORKING CONDITIONS

General Office Environment Community and in home setting during client visits. Travel approximately 70% using a company vehicle. Potential for exposure to blood products, contagious disease, etc.
