The Patient Safety/Risk Manager (PSRM) Registered Nurse (RN) functions as a clinical expert and executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex Veteran care. The PSRM RN works with a team of patient safety managers and the risk manager to ensure cross coverage for both programs and assist with required tasks. Basic Requirements: English Language Proficiency. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Grade Determinations: The following Scope, Education and Dimension criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade. The Dimension requirements (Practice, Veteran/Patient Driven Care, Leadership, Professional Development and Evidence-Based Practice/Research) are detailed for each grade and level within the online assessment:

Online Assessment Physical Requirements: Light lifting, under 15 pounds; Light carrying, under 15 pounds; Walking 1-2hrs; Standing 1-2hrs; Hearing (aid may be permitted). Environmental Factors: Working closely with others; Working alone. The PSRM RN performs duties directly contributing to high quality and safe Veteran care within the VHA. Duties require the ability to apply specialized principles and practices of patient safety science to ensure a systems approach to healthcare delivery. The PSRM RN directly supports facility leadership in areas of planning, organization, development, direction, and evaluation of the patient safety program within RVAHCS. Duties include but not limited to: Functions as a resource person and consultant for risk management, patient safety improvement, and problem-solving activities to ensure that Veterans, staff and leadership needs are provided in a safe environment and in a timely manner. Assists with developing and coordinating facility-wide Patient Safety Improvement activities including but not limited to annual Healthcare Failure Mode and Effect Analysis (HFEMA), annual Safety Risk Analysis of RVAHCS, the annual Patient Safety Plan, PSATs, annual Aggregate Reports to Leadership and VISN, bi-annual Mental Health Environment of Care (MHEOCC), leading root cause analysis, and guiding departments. Provides direct support to the development, maintenance and operation of the patient safety process to include all levels of staff within the facility.
