Primary Purpose
We are seeking a highly skilled and compassionate Physician Assistant to join our Advantmed provider network. In this role, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare and other populations.
Your primary objective will be to assess the overall health and well-being of member beneficiaries to ensure accurate and comprehensive risk adjustment coding, leading to greater value-based care.
Responsibilities
As a Physician Assistant - Risk Adjustment Specialist, you will:
- Perform annual wellness visits and health assessments on a population with chronic conditions.
- Deliver patient health education opportunities.
- Assist in closing quality care gaps (i.e., screenings and labs).
- Collaborate with healthcare teams to enhance care delivery.
Requirements and Qualifications
To be successful in this role, you will need:
- A valid unencumbered PA License for the state you will be working in.
- Previous in-home risk assessment experience is preferred.
- At least 3 years of patient care experience (primary care/adult/geriatric, EMR).
