Maintain open and respectful communication with the nursing team. Keep site forms updated. Coordinate any programs outside the house. Assist with customer access to payee services. Participate in on-call. Qualifications: Must be at least 18 years of age. Two (2) years post high school education in a human service related field or two (2) years of full-time equivalent experience serving persons with disabilities is preferred. Must have a valid driver's license and insurance.
