Prevention Navigator

The TransLink program is an innovative, peer focused initiative designed to significantly reduce the risk of substance use, HIV infection, STI and viral hepatitis infections amongst homeless and unstably housed Transgender women living in San Francisco’s Tenderloin neighborhood.

**Essential Functions**:

- Conduct a comprehensive client assessment, in collaboration with client develop a 6-month care plan
- Conduct consistent client follow-up to assess client progress
- Provide intensive navigation services to include supporting clients make scheduled appointments by accompanying them, advocating during visits, modeling and sharing practices that will assist clients become independent in their medical journey
- Provide peer-based support to clients to include invite/accompaniment to social groups and/or recreational events.
- Link clients to Primary Care Services, behavioral health care and HIV/STD testing through SFCHC on-site services.
- Assist high risk patients to obtain PrEP and adhere to medication regimen.
- Provide linkage to SUD treatment
- Other duties as assigned

**Minimum Qualifications**:

- Some college is preferred but a combination of experience and education will be considered
- At least 1 year experience of working or interpersonal experience with transgender individuals and issues
- Familiarity with issues and concerns of Transgender individuals; Experience in working with multiply-diagnosed patients (HIV, mental health conditions and substance abuse), sexual and gender minorities, immigrants and refugees
- Sensitivity to cultural and ethnic concerns as it relates to substance abuse and HIV;
- At least six months experience with harm reduction, client-centered counseling, and motivational interviewing
- Demonstrated reliability and good interpersonal, organizational, verbal, written communication skills. Must be able to handle multiple tasks and have ability to work professionally and ethically within multi-racial and multicultural settings.
- Be able to provide customer service in an effective, courteous, and friendly manner and meet the needs of diverse clientele
- Role model positive behaviors
- Comfortable and culturally competent in working with LGBTQ communities
- Ability to maintain confidentiality and privacy of persons, documents, data, and communications.
- Excellent written and verbal communications skills in English.
- Fully vaccinated against COVID-19, complete with booster shot(s) and able to show proof of vaccination.
- Up to date with vaccinations, especially seasonal flu shot vaccine.
- Proof of TB clearance (issued within the past 12 months).
