FLSA STATUS:               Non-Exempt-Full Time

TIME COMMITMENT:     40 hours/ week; Required; 8 AM- 4:45 PM; Occasional Weekends and Evenings

SALARY:                         $23.00-25.00 per hour DOE

POSITION SUMMARY

The Caseworker-Coach II- Housing Navigator assists Rachel’s Women’s Center participants in establishing and achieving housing goals, coordinating related services and locating suitable housing options. This team member also helps participants identify their strengths and barriers and provides support overcoming obstacles to success. This may include facilitating connection with behavioral and/or physical health care.

ESSENTIAL FUNCTIONS

  • Assists RWC participants in establishing realistic, attainable housing goals.
  • Engages in Diversion conversations and, when appropriate, conducts a VI-SPDAT (Vulnerability Index-Service Prioritization Decisions Assistance Tool) to connect participants with housing resources
  • Provides case management and coordinates care with community partners to assist participants in accessing services appropriate to their individual needs including behavioral health care and medical care.
  • Assists clients with obtaining essential documents and completing applications to obtain benefits, employment, or housing. Supports residents in overcoming barriers to housing and increasing their income. This may include accompaniment to appointments and advocating with service providers or landlords to overcome clients’ challenges in securing suitable housing.
  • Establishes and maintains positive relationships with landlords and rental agencies offering affordable rental properties.
  • Assists clients with completing and submitting rental applications.
  • Provides group presentations and one-on-one sessions with special emphasis on obtaining housing. Educates clients on the current housing market, housing search options, fair housing laws, and tenant rights.
  • Fosters a trauma-informed, safety-focused, welcoming culture within RWC.
  • Documents all client contacts in ClubHOMS, Clarity HMIS and Catholic Charities tracking systems consistent with established community standards and Agency requirements.

 

QUALIFICATIONS:

  • Minimum of two (2) years of experience providing case management for people who are unhoused.
  • Minimum one year of experience serving people living with mental illness and/or substance use disorders.
  • Associate degree in psychology, counseling, social work, or a related field or equivalent experience
  • Solid working knowledge of relevant best practices including Housing First, Low Barrier Services, Trauma-Informed Care, Motivational Interviewing, Diversion, Homeless Prevention, Progressive Engagement, Conflict Resolution and Harm Reduction
  • Well-developed problem-solving skills and flexible, creative thinking skills with a positive, can-do attitude
  • Must have strong computer skills and the ability to become proficient with database software.
  • Ability to read, interpret and apply documents such as policies, procedures, and safety guidelines.

  • Valid California Driver’s License and automobile insurance in compliance with Agency requirements
  • Successful completion of DOJ/FBI Live Scan and TB Screening. Current CPR and First Aid training or the ability to complete training.

 

PHYSICAL DEMANDS: While performing the duties of this job, the employee is frequently required to stand and walk. The employee is also required to sit and work at a computer. May need to lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision.

 

WORK ENVIRONMENT:  The noise level in the work environment is usually moderate.
