Lvn Hospice Nurse

Overview:
We are seeking a compassionate and skilled Hospice Nurse to join our team. As a Hospice Nurse, you will provide specialized care to patients with life-limiting illnesses and support their families during this difficult time. This is a rewarding opportunity to make a difference in the lives of patients and their loved ones.
**Duties**:
- Provide comprehensive nursing care to hospice patients in accordance with their individualized care plans
- Administer medications, monitor vital signs, and assess patient condition
- Collaborate with the interdisciplinary team to develop and implement patient care plans
- Assist with pain management and symptom control
- Educate patients and families on disease progression, treatment options, and end-of-life care
- Coordinate with other healthcare providers to ensure continuity of care
- Conduct regular assessments and document patient progress
- Assist with discharge planning and facilitate transitions to other levels of care as needed
Qualifications:
- Valid nursing license (RN or LPN)
- Experience in hospice care is preferred but not required
- Familiarity with discharge planning systems and processes
- Proficiency in medical terminology, ICD-9 coding, and electronic health records (eClinicalWorks)
- Strong communication and interpersonal skills
- Ability to provide compassionate care in a sensitive and empathetic manner
- Knowledge of aseptic technique for specimen collection processing
- Reliable Transportation. Valid Auto Liability Insurance
Rate of Pay is negotiable upon discussion with Administrator.
Pay: $35.00 - $40.00 per hour

Weekly schedule:
- Monday to Friday
- Weekends as needed
Ability to Relocate:
