At Houston Hospice, we focus on caring, not curing. We take a team approach to care that includes physicians, nurses, social workers, chaplains, hospice aides, volunteers, and bereavement counselors. We provide hospice service wherever patients and families need us: in their home, in a long-term care facility, or in our inpatient care unit that is based in the Texas Medical Center. Our team of doctors and nurses specialize in managing pain and symptoms that are unique to life-limiting illnesses.
*We are seeking a candidate who would be responsible for directly supervising nurses, home health aides, social workers, and chaplains on our team based out of El Campo, Tx. Conferences with leadership to address staff/patient concerns; maintains patient care updates and standard operating policies/procedures while managing team operations within budgetary guidelines.
*assign staff to new admissions.
* Assure the Plan of Care is updated, executed and effective with appropriate visit frequency.
* Develop and promote positive relationships, acting as liaison between team members, patients, families, agency staff, and facilities to meet patient/family needs.
* Perform ongoing review/assessment of team documentation for comprehensiveness, appropriateness, consistency, regulatory compliance and outcomes. Provide consultation and education for staff, health providers and community.
* Assist with orientation of new staff/students; oversee staff, providing communication, training as needed.
* Rotate administrative on-call duty as scheduled.
* Assists in providing service that fosters the organization’s mission, values, vision and goals.
* Graduate of an accredited school of nursing with current RN license to practice in the state of Texas
* Bachelor’s degree in nursing preferred
* Home health, hospice or public health nursing experience
* Ability to supervise staff and coordinate services
* Five years minimum management experience, hospice/palliative experience preferred.
* Excellent organizational, administrative, interpersonal and communication skills
* Knowledge of applicable State/Federal laws governing hospice licensure
* Ability to travel to multi-work locations during workday
