HOSPICE Community Liaison

Job description
For the the Houston Area:
- Analyze the potential of the company's service area to determine target markets.
- To present agency credentials and obtain patient referrals, we will visit doctor's offices, hospitals, Assisted Living Facilities, Skilled Nursing Facilities, and other possible sources of referrals.
- Analyze the company's organization to determine its strengths and weaknesses.
- Analyze past and current marketing data.
- Complete an analysis of the company's "product" line.
- Analyze patient/company relationships.
- Develop sales/marketing objectives and sales projections.
- Develop a marketing plan, identify priorities, and set a reasonable timetable.
- Implement a marketing plan that stays within the established timetable.
- Review and evaluate the analyses and plan on an established basis.
- Attend IDT meetings as appropriate.
- As needed, assist office staff in going to Physicians' offices to obtain signatures, drop off IDG updates, etc.
- Provide community outreach and education.
Hospice nurses, social workers, chaplains, home health aides, and volunteer visits to the patient at their residence.
An on-call hospice nurse, 24 hours a day.
Medications for pain and symptom management related to the terminal illness.
Durable medical equipment and supplies.
Counseling and spiritual support for the patient and family.
Respite care for the caregiver.
Anticipatory grief support for all family, friends, and loved ones.Company DescriptionWe Provide:\r
