Seeking **EXPERIENCED** Hospice Nurse to join our team and open to travel to the greater Houston area. We need RN to complete Admissions, Visits and Case Management of patients. Responsible for identifying, assessing, planning, coordinating, implementing, monitoring and evaluating options and services across the continuum of care for individuals in assigned case load. As a Hospice Nurse, you will provide specialized nursing care to patients with life-limiting illnesses and support their families during this difficult time. Your role will involve providing physical and emotional comfort, managing symptoms, ordering medications, and ensuring the highest quality of life for patients in their final stages.
- Per Visit rates that include mileage of:_

**RN PAY RATES**:
**$135.00 to $150.00 for an Admission Visit Start of Care (SOC)**

**$95.00 for a Re-certification of Care (ROC)**

**$75.00 for a Nursing Visit (Case Management)**

**Responsibilities**:

- Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs
- Develop and implement individualized care plans based on patient assessments
- Administer medications and treatments as prescribed by physicians
- Monitor patients' condition and document changes in their health status
- Collaborate with interdisciplinary teams to coordinate patient care
- Provide education and support to patients and their families on end-of-life care options
- Assist with pain management and symptom control
- Offer emotional support to patients and their loved ones
- Coordinate with other healthcare providers to ensure continuity of care

**Experience**:

- 1 year of Hospice RN experience
- 1 year of Hospice Kinnser documentation experience
- Plan of Care Development

Pay: $75.00 - $150.00 per hour
