We are seeking an experienced Hospice Registered Nurse (RN) to fill a full-time Case Manager role in Houston area. This position involves managing a range of responsibilities from patient admissions to routine care and recertifications, ensuring high-quality care and support for our patients and their families.

**Role Overview**:
As a Registered Nurse Case Manager, you will be integral in managing patient care from initial admission through ongoing treatment to recertification. Your expertise will be crucial in overseeing the care plans tailored to individual patient needs and ensuring the delivery of compassionate care.

**Key Responsibilities**:

- Conduct admissions assessments and complete all related documentation.
- Perform routine visits to monitor patient progress and adjust care plans as necessary.
- Manage the recertification process to ensure continued care for our patients.
- Coordinate with a multidisciplinary team to provide comprehensive patient care.
- Serve as the primary point of communication for patients, families, and other healthcare providers.
- Supervise and provide guidance to other nursing staff.

**Requirements**:

- Current RN license in Texas.
- Minimum of 1 year of nursing experience in hospice care.
- Proven ability to manage multiple patient cases with compassion and efficiency.
- Exceptional communication, organizational, and leadership skills.
- Familiarity with electronic medical record systems (e.g., WellSky)
