The Cadence Health team seeks a Chronic Care Health Coach to help provide patient care remotely alongside our Registered Nurses and Nurse Practitioners.

**This role will be required to work Mon-Fri 8 am-5pm MST, CST or PST Hours.**

**WHAT YOU'LL DO**:

- Support delivery of health care to patients by performing a variety of activities and procedures which are prescribed by and performed under the direction of the Cadence Nurse Practitioner and Cadence clinical policies and procedures.
- Provide one-on-one coaching and support to patients managing chronic conditions, including but not limited to type 2 diabetes, hypertension, and cardiovascular disease.
- Conduct comprehensive assessments of patients' health status, lifestyle behaviors, nutritional habits, and readiness to change.
- Help patients execute on their personalized care plans and goals, focusing on behavior modification, nutrition, physical activity, and self-management strategies.
- Monitor patients' progress, adherence to treatment plans, and health outcomes through regular check-ins and remote monitoring tools.
- Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies.

**WHAT YOU'LL NEED**:
**Education and/or Experience**:

- Registered Dietician, Registered Nutritionists, or licensed Social Worker.

**Knowledge and skills**:

- Highly skilled in behavioral based coaching
- Excellent clinical acumen.
- Excels at patient support and delivering a high level of service.
- Exceptional written, verbal, and interpersonal communication skills.
- Prior experience supporting patients remotely is a plus.
- Organized and able to set priorities.
- Works effectively with minimum supervision.
- Works well as a team member.
- Patient assessment competency.
- Patient education.
- Tech fluency and capability to work in multiple systems.
