About the Role

We are seeking a highly skilled Registered Nurse Care Manager to join our team at Summit Health, Inc. As a key member of our care management team, you will play a critical role in delivering high-quality, patient-centered care to our patients.

Key Responsibilities
  • Partner with primary care physicians and other healthcare professionals to develop and implement comprehensive care plans for patients with complex medical conditions.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and identify opportunities for improvement in their care.
  • Develop and implement strategies to address barriers to care, including medication adherence, social determinants of health, and health literacy.
  • Collaborate with patients, families, and caregivers to educate them on their conditions, treatment options, and self-management techniques.
  • Provide coaching and support to patients to help them achieve their health goals and improve their overall well-being.
  • Work closely with our pharmacy, social work, and payer partners to develop and implement focused intervention programs for patients with chronic conditions or complex social or behavioral needs.
  • Identify and address gaps in care across our empaneled population, and develop strategies to improve care coordination and reduce avoidable utilization.
  • Develop and maintain effective professional working relationships with assigned primary care practices and hospital systems.
  • Engage patients in a variety of settings, including clinics, homes, facilities, and community settings, to ensure they have an optimal care experience and maintain connection to their primary care provider.
Requirements
  • 5+ years of direct, acute care clinical nursing experience, preferably in emergency medicine, med/surg, or telemetry.
  • Registered Nurse with an unincumbered license in the state of practice.
  • Care management experience in a primary care or inpatient setting preferred.
  • Valid driver's license and personal transportation for community or hospital visits.
  • Comfort and efficiency with technology, including Microsoft suite of products, and electronic health records.
