Job Summary

We are seeking a skilled Physician to join our team as a Chronic Care Management Specialist. In this role, you will be responsible for establishing the Tapestry Health patient relationship for our chronic care management services.

Key Responsibilities

  • Establish and maintain strong relationships with patients and their families
  • Document patient encounters accurately and thoroughly within our platform at time of visit
  • Active, unrestricted, Medical Licenses in Texas
  • Must have reliable transportation to travel to assigned buildings
  • Commitment to providing patient-centered care and improving health outcomes
  • Familiarity with electronic health record (EHR) systems and healthcare technology
  • Ability to travel within 1-2 hours of assigned region

Compensation Structure

We offer a competitive compensation structure, including:

  • $25 per patient visit, paid upon completion of assigned facility census
  • Mileage reimbursement available
