Position Summary

In this contractor role, the Medical Doctor will be responsible for establishing the Tapestry Health patient relationship for our chronic care management services. Located in Austin, Texas.

Key Responsibilities
  • Establish the Tapestry Health patient relationship for chronic care management services.
  • Conduct approximately 375 clinical visits onsite, with a minimum of 12 working days per month.
  • Document patient encounters accurately and thoroughly within our platform at time of visit.
  • Provide end of day report to program administrators to track progress for completion of visits.
Minimum Requirements
  • Qualified MD/DO
  • Active, unrestricted, Medical Licenses in Texas
  • Excellent clinical judgment and decision-making skills.
  • Reliable transportation to travel to assigned buildings.
  • Commitment to providing patient-centered care and improving health outcomes.
  • Familiarity with electronic health record (EHR) systems and healthcare technology.
  • Ability to travel within 1-2 hours of assigned region.
Compensation Structure

$25 per patient visit, paid upon completion of assigned facility census. Mileage reimbursement available.
