Program Operations Specialist Trainee 1: A bachelor's degree in a human services field.
Program Operations Specialist Trainee 2: A bachelor's degree in a human services field and one year of experience in a federal or state certified human service program, which must have been in a professional administrative position in which a major duty involved planning, coordinating, monitoring or evaluating a provision of services for people with developmental disabilities OR a Juris Doctor, master's or higher degree in a related field can substitute for one year of specialized experience. This experience must have included information gathering and analysis.

**Duties Description**

The Incident Management Unit (IMU) within the Division of Quality Improvement was created as part of the initiative to ensure the safety and welfare of individuals with developmental disabilities. This position will provide coverage for the IMU's after-hours call-in system. The Program Operations Specialist 1 assigned to this shift will:

- Provide real-time oversight of the critical function of incident management on a statewide basis. Ensure appropriate information is provided for each initial report of serious reportable incidents, allegations of abuse and all deaths. This includes ensuring adequate protective measures for the individuals are taken, that the incidents are classified correctly, and a thorough description of each incident has been received, and that required notifications to entities such as law enforcement are completed as appropriate.
- Perform routine checks of the Incident Notification mailbox are done and that follow up contacts for the individuals are reported directly back to provider agencies as necessary.
- Review of the Incident Notification mailbox for incidents generated by the Justice Center for the Protection of People with Special Needs that require assignment and completion.
- Generate Critical and Significant Event reports based on established criteria on an ongoing basis. Respond to inquiries to related incident management as appropriate.
- Review of investigations and investigative records. Processing of investigative and related incident records as required. Interaction with oversight agencies and provider agencies relative to investigative records procedures and process and follow up as necessary.
- Work in conjunction with the Office of Investigations and Internal Affairs and the Office of the Employee relations, as well as the Statewide Committee on Incident Review (SCIR) and Local Incident Review Committees, to ensure a strong, coordinated approach is taken in response to any abuse allegations or other serious incidents that may occur within state-operated services.
- Conduct initial review of individual incidents of mortality, and additional incidents as assigned, that are reported in the Incident Report and Management Application (IRMA). Using criteria developed by the Central Mortality Review Committee, the POS 1 will make appropriate recommendations for each incident. For mortality incidents recommended for review and closure, the POS 1 will submit to the Local and Central Mortality Review Committees a summary of each case and aggregated data regarding all cases recommended for closure.
- Provide ongoing technical assistance and direction to both state operated and voluntary provider agencies in the area of incident management related to deaths.
- Assist in the development of appropriate and standardized trend analysis reports for the Commissioner's and Leadership Team's review related to deaths.
