**Qualifications**:

- Bachelor's degree in Social Work, Psychology, or related human services field.
- Minimum of three years of experience working with at-risk youth.
- Must be 21 years old.
- A valid driver's license with a clean standing over the past 6 months is required.
- Physical & Drug Screening, Criminal Record History, FBI, and Child Abuse History Clearances.
- All new hires are required to successfully complete Safety Crisis Management (SCM) certification within one week of hire.

- **Compile Information**:

- Gather all relevant information and documentation related to youth.
- **Service Coordination**:

- Participate in service coordination on behalf of youth and families under the supervision of the MDT and/or licensed therapist.
- **Collaboration with MDT**:

- Collaborate with licensed therapists, youth, family/guardians, and MDT members to provide care coordination input in the development of individual services plans, treatment plans, and IMDS tools.
- **Liaison Role**:

- Serve as a liaison for school, community, and ancillary services.
- **Representative Role**:

- Represent the youth and family on the agency's behalf in screening, placement, court appearances, and other relevant situations.
- **Consistent MDT Contact**:

- Remain in consistent contact with all members of the youth's MDT, ensuring seamless communication and collaboration.
- **Youth-Centered Advocacy**:

- Provide youth-centered advocacy, championing the needs and rights of the youth and their families.
- **Referral and Linkage**:

- Provide referral and linkage to ancillary and specialized treatment services and interventions to address specific needs.
- **Site Visits**:

- Routinely conduct school, home, and ancillary visits to gain a comprehensive understanding of the youth's environment.
- **Special Appointments**:

- Schedule and attend special appointments, facilitating necessary services and support for the youth.
- **Correspondence Management**:

- Take responsibility for correspondence on behalf of the youth and family, including treatment team meeting invitations, as well as monthly/quarterly plan dissemination to MDT members.
- **File Management**:

- Ensure effective file management, maintaining accurate and up-to-date records of youth and family information.
