JOB SUMMARY AND QUALIFICATIONS:
The primary responsibility of the Targeted Case Manager is to deliver case management services to families engaged in the Family Recovery Program. The Case Manager will assist individuals and families in accessing essential services, including housing, medical, social, educational, and other identified needs.
REQUIREMENTS FOR THIS ROLE:
A Bachelor's degree in a behavioral science (such as psychology, sociology, social work, family studies, human services, counseling, or another approved human service degree) is required. Candidates should have a minimum of one (1) year of full-time experience working directly with adolescents or adults in a human service environment, preferably in the field of Substance Use. Additionally, successful completion of case management training approved by the Kentucky Department for Behavioral Health, Developmental and Intellectual Disabilities (KBHDID) is necessary, along with recertification every three years. Candidates should possess knowledge and skills in case management, crisis counseling, community resources, advocacy, problem-solving, critical thinking, communication, mental health, substance use, and documentation. A Master's degree in a behavioral science is preferred.
KEY RESPONSIBILITIES:
1. Provide case management services tailored to the specific needs of individuals and families, with some services delivered in the client's home.
2. Conduct thorough assessments and reassessments to identify the need for housing, medical, social, educational, and other services.
3. Develop personalized case plans for each individual and family based on assessment findings, outlining goals and activities related to housing, medical, social, educational, and other services.
4. Facilitate referrals to appropriate services based on assessment outcomes, including scheduling appointments and ensuring transportation is arranged.
5. Monitor and follow up on established goals and activities in the case plan, maintaining communication with clients, family members, and service providers, and updating the case plan as necessary.
6. Identify community resources and cultivate collaborative relationships with other social service providers.
7. Accurately represent agency programs, policies, and procedures within the community and to other social service organizations.
8. Maintain precise case records, ensuring proper documentation of all client interactions.
9. Ensure compliance with KY Medicaid and accreditation standards in all documentation.
10. Conduct Life Skills classes for identified individuals in need of this service.
11. Participate in weekly case conferences, providing updates on all assigned individuals and families.
12. Engage in bi-monthly supervision with the TCM billing Supervisor.
13. Attend monthly staff meetings and lead training sessions or discussions as assigned.
14. Complete the annually required 10-hour SUD training to maintain specialty certifications.
15. Uphold certification requirements for TCM as per Kentucky Board regulations.
16. Assist with special projects as assigned.
17. Support reporting requirements as assigned.
