This position requires working in the community and performing home visits to provide support and services to individuals with mental illness.

The Case Manager coordinates all necessary services to maximize consumer recovery and independence, implementing strategies for obtaining services and supports that are goal-oriented and individualized.

Key responsibilities of the Case Manager include:

  • Ensuring consumer involvement in the Person Centered Planning Process (PCP)
  • Facilitating the implementation of Individual Plan of Service
  • Performing comprehensive, ongoing psychosocial assessments
  • Participating in legal proceedings, including petitioning and testifying in clinical emergencies
  • Providing community-based services
  • Facilitating the coordination of consumer intakes, discharges, and transfers
  • Communicating with all service providers in Treatment Plan
  • Facilitating hospitalizations, attending hospital meetings, and coordinating with hospital social worker
  • Coordinating and monitoring activities such as a job, school, volunteerism, and family caretaking
  • Providing referral to appropriate community resources

To be successful in this role, you will need:

  • Bachelor's degree in a human services field from an accredited institution
  • LLBSW, LBSW, LLMSW, LMSW preferred
  • Prior case management experience in a mental health or rehabilitation setting preferred
  • Prior experience working with Developmentally Disabled population required
  • Valid Michigan Driver's License
Education

Required:

  • Bachelor of Arts or better in Criminal Justice or related field
  • Bachelor of Social Work or better
  • Bachelor of Human Services or better
  • Bachelor of Science or better in Psychology or related field
  • Bachelor of Science or better in Criminal Justice or related field
