Job Summary

We are seeking a highly motivated and compassionate Case Manager to join our team at Hope Network New Passages. As a Case Manager, you will play a critical role in supporting individuals with mental illness to achieve their recovery goals.

Key Responsibilities
  • Coordinate all services necessary to maximize consumer recovery and independence, including implementing strategies for obtaining services and supports that are goal-oriented and individualized.
  • Ensure consumer involvement in the Person-Centered Planning Process (PCP) and facilitate the implementation of Individual Plans of Service.
  • Perform comprehensive, ongoing psychosocial assessments and participate in legal proceedings, including petitioning and testifying in clinical emergencies.
  • Provide community-based services, facilitate the coordination of consumer intakes, discharges, and transfers, and communicate with all service providers in the Treatment Plan.
  • Facilitate hospitalizations, attend hospital meetings, and coordinate with hospital social workers, as well as coordinate and monitor activities such as employment, education, volunteerism, and family caretaking.
  • Provide referrals to appropriate community resources and act as a positive role model and coach for others.
Requirements
  • Bachelor's degree in a human services field from an accredited institution required.
  • LLBSW, LBSW, LLMSW, LMSW preferred.
  • Prior case management experience in a mental health or rehabilitation setting preferred.
  • Prior experience working with Developmentally Disabled populations required.
  • Valid Michigan Driver's License.
