**Program Summary**

Arise: Asian Wellness Project, an Underserved Ethnic Language Population (UELP) program provides services to the East Asian population in South Alameda County. Our goal is to promote emotional and mental well-being through education, consultation, and outreach to historically underserved ethnic and language populations.

**Overall Summary**:
The Community Health Worker (CHW) I at Bay Area Community Health will primarily work at serving and assisting our patients while located at different sites throughout the organization and community events. The Community Health Worker (CHW) will work at different capacities based on their assigned area of training and specialty. The CHW will be a trusted member of the community, and actively participate as a liaison between primary care, specialties and resources available to BACH patients. Activities may include, but are not limited to, outreach, engagement, care coordination, case management, patient education, health coaching, linkage of the patient with resources at BACH and in the community such as food, housing, financial and transportation assistance and also linking the patient to care within the clinic.

**Essential Responsibilities**
- Working out in the community, determines the eligibility of participants for mental health services through interactive interviewing and fact gathering; maintains current knowledge of program.
- Provides outreach, engagement, education, screening, and referral services with goal of reaching the target population and getting them connected to prevention services
- Collaborate with other staff to create personalized wellness plans that include traditional healing practices and cultural aspects, provide support groups, as well as follow-up with participants to prevent escalation of mental health crises.
- Develops, implements, and translates bi-lingual health education materials.
- Maintains accurate records and submit program reports as required.
- Participates in health fairs, presentations, support groups, and community outreach events.
- Assists in the development and implementation of outreach strategies.
- Enhance service program awareness through Social Media marketing.

**Secondary Responsibilities**
- Attends workshops, training and meetings as needed, and as requested.
- Other related duties as assigned by supervisor.

**Qualifications**:
**Required education, experience, training**
- Must be Bilingual/Bicultural Korean, Mandarin, Cantonese or Japanese.
- College degree or equivalent education and experience.
- Six months customer service experience with the demonstrated ability to work with people of diverse backgrounds with respect to race, ethnicity, class, sexual orientation and disability.
- Experience conducting outreach activities and presentations within Asian communities.
- Self-starter, able to work independently.
- Demonstrated ability to show non-judgmental attitude and maintain client confidentiality.
- Access to reliable transportation.
- Proficient in Microsoft and Internet.

**Preferred education, experience, training**
- Current knowledge of community agencies and services in Alameda County.
- One-year experience in health or human service agency.
- Experience working with youth or Asian populations.

**Physical Demands**

This position requires:

- Sitting, walking, and standing associated with a normal medical/clinical office environment.
- Regular bending and stretching; and lifting.
- Manual dexterity used to include, but not limited to, standard office equipment, and computer keyboard.
- Traveling to work sites including some exposure to outside elements.
- Presenting information in front of groups or public.
- Reading and writing in order to review records for accuracy.
