**Position Function**:
Provide housing support and case management services to Montgomery County residents experiencing homelessness and/or a housing crisis. Services will focus on rapid re-housing, housing stability, and intensive case management by providing access to supportive services and community resources. Be responsible for helping to maintain relationships with landlords, property managers, and housing inspectors to strengthen permanent housing opportunities for program-eligible families and individuals served through the Norristown Housing Resource Center.

This is a grant-funded position and is dependent upon the renewal of the grant.

**Qualifications**:
Must meet **ONE** of the following:

- Bachelor’s degree in social work with 2+ years of experience in shelter, transitional, or supportive housing
- Bachelor’s degree in another related field with 3+ years of experience in shelter, transitional, or supportive housing
- Master’s degree in social work or a related field

Must have a valid driver’s license, daily access to a reliable vehicle, and car insurance

Preferred but not required: Bilingual in English and Spanish

**Essential Job Functions**:

- Complete intake interviews and assessments to determine program eligibility.
- Effectively collaborate with Your Way Home affiliates, shelter providers, internal programs, and other community resource providers to meet client needs.
- Develop, increase, and monitor the tenancy skills of the individuals and families served.
- Provide coaching to ensure domestic violence victims and survivors are aware of their rights and responsibilities with respect to housing.
- Meet regularly with individuals and families in their housing units and in the community.
- Complete all required client-related written and electronic documentation in compliance with programmatic requirements and agency guidelines.
- Actively participate in Your Way Home meetings, trainings, and learning collaboratives.
- Actively participate in weekly supervision and Norristown Family Center program meetings.
- Comply with the Your Way Home policy manual.
- Determine client’s rental contributions and create a consumer-driven Housing Stability Plan (HSP).
- Follow-up with families and individuals as recommended in the YWH policy to provide guidance and support with progress on their HSP.
- Maintain client information in the Clarity Human Services (HMIS) database.
- Complete other duties as assigned.

**Knowledge, Skills, and Abilities**:

- Understanding of the homeless population and fair housing laws
- Knowledge of child welfare, child and family service systems, and social work values
- Understanding of principles of mental illness and substance abuse as well as the ability to implement them with behavioral best practice and crisis management planning
- Strong problem-solving, conflict resolution, and communication skills
- Ability to conduct outreach with and engage community members and landlords
- Ability to use initiative, act independently, and effectively manage time
- Ability to work within a team structure
- Personal safety skills
- Good oral and written communication skills
- Basic math skills

**Work Environment and Primary Work Locations**:

- CVCA Norristown Office
- 5 workdays per week with flexible schedule
- Occasional weekday evenings until 8 P.M.
- Occasional weekend hours
- 25% office-based work; 65% traveling to conduct home and community visits throughout Montgomery County; 10% attending community meetings, trainings, and events

**Physical/Mental Considerations**:

- Ability to lift and carry 20 lbs.
- Ability to climb stairs
- Ability to maintain composure in crisis situations
