ACE’s Prevention Program: ACE’s Prevention program serves youth, ages 10-15, who are gang affected, otherwise at risk of gang involvement, or system involvement. Utilizing a comprehensive approach,  participants and their families engage in programming that leads to social-emotional development, that strengthens participants' family ties, increases protective factors, and promotes other pro-social influences.
Position: The Care Manager is responsible for providing services to participants and their families of the 6-month GRYD Prevention program. This position is involved with all aspects of the program including community outreach, intake and enrollment, individual and family case management, facilitating individual and group activities, and documentation.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Knowledge of community/gang dynamics within the GRYD Zone.
  • Able to operate within ACE’s designated GRYD zone, exposing oneself to gangs found within the zone.
  • Able to work in assigned locations that may expose oneself to hazardous conditions or in extreme physical discomfort or distress.

DUTIES AND RESPONSIBILITIES:
  • Attends and participates in all mandatory meetings.
  • Fosters positive relationships with community members and partner organizations.
  • Participate in cultural and community-building activities/events.
  • Works to incorporate into organizational culture best practices for diversity, equity, inclusion, restorative justice, and trauma-informed care. 
  • Performs outreach and marketing duties to recruit participants and educate the community on ACE Services.
  • Carries out intake and eligibility determinations and makes appropriate referrals when the applicant is not eligible for ACE services; conducts participant assessments leading to enrollment; completes and updates participant service plans. 
  • Arrange and participate in participants’ family meetings, individual meetings, team meetings, case consults with GRYD Intervention, etc., and coordinate wrap-around services with other providers; provide transportation of program participants to meet program goals.
  • Organizes, prepares and facilitates intentional youth development and skills-based workshops for participants.
  • Enters data into participant databases (organizational and program); assists in the writing of periodic program reports.
  • Provides case management for an average standing caseload of twenty-five participants.
  • Responsible for administering eligibility assessment.
  • Responsible for inputting participant information and case notes into appropriate database(s).
  • Responsible for assisting participants and their families in making referrals to partner organizations.
  • Responsible for organizing celebration activity meetings with participants and support systems.
  • Participates in all GRYD community-based events including Saturday Night Lights and Fall Friday Night activities.
  • Conducts court advocacy as needed for program participants. 
  • Plan and coordinate Field trips and provide onsite supervision. 
  • Address and hold youth accountable for program expectations through a trauma-informed lens.
  • Other duties as assigned.

PHYSICAL REQUIREMENTS:
  • Prolonged periods of standing and walking throughout facilities.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
  • Must be able to bend, lift, stretch, climb, and crawl to provide services.

QUALIFICATIONS:
  • Bachelor’s Degree in a relevant field and 2+ years of experience working with youth; or an Equivalent combination of training and experience.
  • Spanish language skills, verbal and written required. 
  • Possess a current California Driver’s License, clean DMV record & adequate auto insurance. 
  • Must submit to a Department of Justice Criminal background check. 
  • Initial TB employment clearance required.

The anticipated pay range for candidates who will work in California is $28.85 to $30.45 hourly.  The offered pay to a successful candidate will be dependent on several factors that may include but are not limited to years of experience within the job, years of experience within the required industry, education, etc.
