Client Coordinator

WellLife Network is seeking a dedicated Client Coordinator to join the team at our mental health facility in Queens village. The ideal candidate will provide exceptional direct support care to our residents while playing a crucial role in maintaining their medical records.
Some Essential Task:
  • Must fulfill all the requirements of the Direct Support Professional role including, but not limited to, working at developing independent living skills, administering medications, promoting opportunities for choice, participating in recreation activities, and any other tasks required to ensure the safety and well-being of the consumers.
  •  Will track all medical appointments and follow-up appointments 
  • Ensure that medication inventories are completed and order new medication as prescribed, in a timely manner.
  • Identify medication errors and communicate the errors to the nurse and manager effectively
  • Will perform any other duties as assigned by a supervisor.

Education and Experience Requirements
  • NYS valid and clean Driver license
  • HS Diploma or GED
  • 1 year of HHA, CNA experience a plus
  • 2 years of Direct Support Professional experience a plus
  • Nursing student preferred
