Job Purpose: The CST Qualified Professional is responsible for providing CST services according to CST service definition and supervision requirements stipulated in Rule 10A NCAC 27G .0104. This position involves delivering intensive community-based rehabilitation team services, including direct treatment, restorative interventions, and case management.

Major Duties and Responsibilities (Under the direction of the Team Lead):
  • Supervision: Ensure supervision according to licensure and certification requirements of appropriate disciplines.
  • Team Participation: Participate in weekly case staffing meetings.
  • Psychoeducation: Provide psychoeducation as indicated in the Person-Centered Plan (PCP).
  • Monitoring and Evaluation: Assist the Team Lead in monitoring and evaluating the effectiveness of interventions, evidenced by symptom reduction and progress toward goals identified in the PCP.
  • Consultation: Consult with identified providers, engage community and natural supports, and include their input in the person-centered planning process.
  • Crisis Interventions: Assist with crisis interventions 24/7/365 as indicated in the Individual’s crisis plan.
  • Behavioral and Substance Use Interventions: Assist with behavioral and substance use disorder treatment interventions.
  • Plan Development and Implementation: Complete the initial development and ongoing revision of the PCP and ensure its implementation.
  • Clinical Oversight: Provide clinical oversight and coordination of Associate Professionals and Paraprofessionals in implementing interventions in accordance with the PCP.
  • Collaboration: Work closely with other clinical/professional staff to maintain communication and provide feedback, standardize procedures, and expedite PCP implementation.
  • Medical and Non-Medical Consultation: Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., DSS, school, DOJ) providers, engage community and natural supports, and include input in the person-centered planning process.
  • Service Linkage: Ensure linkage to the most clinically appropriate and effective services, including arrangements for psychological and psychiatric evaluations.
  • Behavioral Health Services: Provide and coordinate behavioral health services and other interventions for the Individual.
  • Functional Needs Assessment: Complete functional needs assessment(s) to determine the scope and anticipated outcomes of the services.
  • Relapse Prevention: Assist with the development of relapse prevention and disease management strategies.
  • Case Management: Provide and assist with intensive case management, including linkage and arrangement for services and referrals, monitoring, and follow-up.
  • Housing Assistance: Assist Individuals with housing searches, engage landlords, write reasonable accommodation letters, connect Individuals to financial and in-kind resources, prevent and mitigate housing crises, and assist with rehousing if necessary.
  • Daily Living Skills: Assist in developing daily living skills to stabilize and maintain housing.
  • Staff Ratio: Maintain an Individual-to-staff ratio of 12:1 with a team maximum of 48 individuals.
  • Confidentiality and Compliance: Maintain confidentiality and comply with governmental laws and regulations, as well as Company policies and procedures.
  • Team Contribution: Complete any other duties and responsibilities as assigned by SEBHS Director(s), Manager(s), Supervisor(s), and/or other SEBHS Authorized staff.
Training Requirements:
  • Core Training (Must be completed prior to hire date):
    • Current First-Aid and CPR.
    • Blood Borne Pathogens (BBP).
    • Restrictive Interventions Certifications.
  • Within 30 Days of Hire:
    • Agency required in-house trainings.
    • CST Service Definition Training/Clinical Coverage Policy - 3 hours.
    • Crisis Response Training - 3 hours.
    • PCP Instructional Element Training - 3 hours.
    • CST Service Note Documentation Training - 3 hours.
  • Within 90 Days of Hire:
    • Introduction to Motivational Interviewing (MI) Training by a MINT Trainer (2 days) - 13 hours.
    • Person-Centered Thinking Training from a PCT Trainer – 12 hours.
    • Permanent Supportive Housing Training – 15 hours.
    • Basics of Psychiatric Rehabilitation and Functional Assessment – 3 hours.
    • Trauma Informed Care – 3 hours.
    • Designated therapies, practices or models specific to the population(s) to be served by each CST Team (e.g., Cognitive Behavior Therapy, Trauma-Focused Therapy, Illness Management and Recovery - SAMHSA Toolkit) - 12 hours.
  • Annually: Follow up training and ongoing continuing education required for fidelity to chosen modality (minimum of 10 hours if no requirements are designated by developers of that modality).

Minimum Qualifications:
  • Education/Experience:
    • Must meet the requirements for Qualified Professional and have the skills, knowledge, and abilities required by the population and age to be served.
    • Minimum of one (1) year documented experience working with MH/DD/SA population.
    • Specific credentials required depending on the level of education and experience.
  • Additional Requirements:
    • Valid Driver’s License, Auto Insurance, and reliable transportation.
    • Criminal/DMV background check.
    • Available to accept random drug testing.

  • Technical Skills: Proficiency in using computers and relevant software.
Agency Responsibilities:
  • Attendance at required meetings and trainings.
  • Documentation of clinical work and maintenance of medical records.
Level of Authority:
  • Perform daily routines independently with sound judgment and competence.
Working Conditions:
  • Work within the facility, outside the facility, community, school, and/or within the Individual’s home.
  • Flexible hours required.
  • Provide “first responder” crisis response on a 24/7/365 basis.
  • Travel using personal vehicle required.
