Community Support Team

Who We Are Looking For: We are seeking a skilled and empathetic Community Support Team (CST) Qualified Professional (QP) to join our team. This role is pivotal in delivering high-quality mental health and substance use services to help clients achieve their recovery goals and improve overall well-being.
What You Will Do:
- Conduct comprehensive assessments to identify client strengths, needs, and goals.
- Develop, implement, and monitor tailored treatment plans in collaboration with clients, their families, and CST team members.
- Provide direct therapeutic interventions, including individual, family, and group therapy, as well as crisis intervention.
- Coordinate and link clients to necessary community resources.
- Advocate for clients within various systems to ensure comprehensive support.
- Regularly monitor client progress and adjust treatment plans to ensure effectiveness.
- Maintain detailed documentation in accordance with agency and regulatory standards.
- Collaborate closely with CST team members to ensure a holistic approach to client care.
- Participate in team meetings and case conferences to discuss and coordinate client care.
Additional Responsibilities:
- Provide psychoeducation as indicated.
- Assist with crisis interventions and the development of relapse prevention and disease management strategies.
- Coordinate assessment activities and ensure the implementation of the person-centered plan (PCP).
- Consult with medical and non-medical providers, engaging community supports and incorporating their input into the planning process.
- Link clients to clinically appropriate services and arrange for necessary evaluations.
- Complete functional needs assessments to determine service scope and anticipated outcomes.
Qualifications:
- A Bachelor's degree in a related field.
- At least 2 years of experience working with individuals with mental health or substance use challenges, preferably in a community-based setting.
- Strong knowledge of evidence-based practices in mental health and substance use treatment.
- Excellent communication, organizational, and problem-solving skills.
- Commitment to person-centered care and cultural competence.
- Valid driver’s license and reliable transportation.
