Position Summary

The Care Coordinator (CC) is an integral part of the inter-disciplinary Care Management team, and will assist in facilitating patient continuity of care for GLIN’s highest-risk patients. Under the clinical direction and oversight of the Nurse and Social Work Care Managers, the Care Coordinator will assist at-risk patients in various ways with the goal of improving overall patient health. To address patient needs and expedite appropriate, cost effective care, the Care Coordinator will assist with telephonic outreaches to patients to engage and enroll in GLIN Care Management programs, and will support the Care Management team with needed provider and community agency collaboration. The Care Coordinator will also be responsible for administrative/clerical tasking such as preparing patient Electronic Medical Records (EMRs), requesting/uploading/downloading patient health information, faxing and mailing letters, and answering inbound calls to the Care Management (CM) queue.

Duties and Responsibilities (including but not limited to)

  • Address patient obstacles to care
  • Facilitate engagement between PCP and patient
  • Daily collaboration with Care Management Team
  • Work as patient advocate
  • Performs additional duties as assigned
  • Answer inbound calls to Care Management queue
  • Place outbound and/or engagement calls as needed
  • Prepare patient charts for potential outreach
  • Establish patient eligibility for CM programming
  • Utilize multiple systems to gather information
  • Document all communications appropriately in EMR

Qualifications or Education, Training and Experience

  • High school or GED diploma
  • Two (2)+ years of work experience in one or more of the following settings: Acute Care, Ambulatory, Specialty, Physician Office, Care Management, Home Health Care or related
  • Must have ability to travel to and from office for required training/ meetings

Knowledge and Skills/Expected Competencies

  • Multitasking and prioritization working in a high-volume environment
  • Understanding and implementation of HIPAA and Confidentiality requirements
  • Excellent interpersonal and organizational skills
  • Compassionate and patient centric approach
  • Adaptable to new technologies and software
  • Proficiency (including documenting in) in EMR system(s), Outlook, basic Excel


*This is a hybrid-remote position and candidates must reside in WNY to be considered.

Pay range $18.00 to $24.00

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within job’s compensation range, and will be determined by considering factors including, but not limited to market data, education, experience, qualifications, and expertise of the individual, and internal equity considerations.
