Overview of Role & Primary Responsibilities
The Clinical Liaison (RN) will serve as clinical team lead and will report to the Senior Director of the Virtual Care Center. This individual will lead and optimize patient enrollment, orient and educate the care team and patients to Reimagine Care services, and provide clinical care at the client’s designated sites. This individual must strongly align with Reimagine Care’s core values, which focus on prioritizing patient and colleague respect and care, working collaboratively while demonstrating initiative and resourcefulness, and advancing our innovation and impact to internal and external stakeholders.
Core Responsibilities: This will be a tiered position, with each piece of the job description building on the other:
Tier 1: Development
- Build a positive working relationship and be an essential point of contact and clinical resource for patients, providers, and care teams.
- Work to standardize, refine, and improve workflow processes for patient, provider, and care team engagement and satisfaction to optimize patient enrollment, engagement, and satisfaction.
Tier 2: Maintenance
- Review and analyze patient enrollment, patient experience, and provider experience to identify trends, gaps, and areas for improvement.
- Initiate, engage, and collaborate with the client’s operational and clinical teams to understand/evaluate business and clinical needs aligned with established goals and metrics.
Tier 3: Clinical care
- Work collaboratively with members of the Virtual Care Center to provide patient care and follow all Reimagine Care policies, procedures, pathways, and communication and documentation best practices.
- Understand and adhere to approved symptom management pathways, protocols, patients’ severity level, care pathway, and evaluation timeframes.
Education, Experience, and Skills
Required:
- Education, Licensure, and Experience:
- Bachelor of Science in Nursing (BSN)
- Current and valid Registered Nurse license in the state of practice
- Minimum of 5 years of clinical experience in oncology
- Skills:
- Strong quality and patient/client-centered focused
- Ability to work within a team and as an individual contributor
- Comfortable in a fast-paced, changing environment
- Ability to work in a self-directed environment
- Excellent interpersonal skills and ability to work with a wide range of individuals
