ABOUT THIS ROLE

As an Intensive Care Coordination Specialist, you evaluate referrals and promptly reach out to families. You conduct assessments to understand their strengths and needs, using IM+CANS to create and update personalized service plans. You develop and maintain Crisis Prevention and Safety plans, help families identify support networks, and facilitate Child and Family team meetings every 60 days or after a crisis. Engaging with families often, you can link them to recommended services and collaborate with MCO Case Managers and other providers. Managing up to 25 families, you maintain detailed case records and provide strength-based, culturally competent, and trauma-informed care, making a meaningful impact daily.

WHAT WE'RE LOOKING FOR IN AN INTENSIVE CARE COORDINATION SPECIALIST

We are searching for someone who is detail-oriented, organized and has great communication skills. You might be perfect for this position if you can also meet the following qualifications:

  • Bachelor's degree in counseling and guidance, rehabilitation, social work, education, psychology, or related human service field, along with two years of supervised experience
  • Ability and willingness to actively participate in a fast-paced, multidisciplinary team environment
  • Knowledge of community programs, services, evidence-based practices, and a variety of treatment modalities
  • Possess and maintain a valid driver's license and provide proof of insurance
  • Strong time management skills
  • Excellent interpersonal and written communication skills
  • Must possess the knowledge, personal qualities, and practice skills to work effectively with professionals in other organizations and from other disciplines
