BECOMING OUR INTENSIVE CARE COORDINATOR

As an Intensive Care Coordinator, you'll play a crucial role in providing comprehensive care coordination services to youth and families facing complex challenges. You'll evaluate referral information, conduct assessments, and develop individualized service plans. By facilitating regular meetings with Child and Family Teams, you'll ensure effective communication and collaboration among stakeholders. Your dedication will help families access resources, navigate systems, and achieve their goals, making a lasting impact in their lives.

All that we need from you is the following:

  • Bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, psychology, or a related human service field
  • 2 years of supervising experience
  • Knowledge of community programs, services, evidence-based practices, and various treatment modalities used to work with youth and their families to navigate child-serving systems
  • Ability to actively participate in a fast-paced, multidisciplinary team environment
  • Valid driver's license and proof of vehicle insurance
  • Strong organizational and time management abilities
  • Excellent interpersonal and written communication skills
