WHAT'S YOUR DAY LIKE?

As a Critical Care Coordinator, you play a crucial role in the Care Coordination and Support program. Your responsibilities include evaluating and interpreting referral information, conducting strength needs assessments, and creating individualized service plans. WHAT'S NEEDED FROM A CRITICAL CARE COORDINATOR?

We're looking for someone who can work a full-time schedule while meeting the following qualifications:

  • Bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, psychology, or a related human service field
  • 2 years of supervising experience
  • Knowledge of community programs, services, evidence-based practices, and various treatment modalities used to work with youth and their families to navigate child-serving systems
  • Ability to actively participate in a fast-paced, multidisciplinary team environment
  • Valid driver's license and proof of vehicle insurance
  • Strong organizational and time management abilities
  • Excellent interpersonal and written communication skills
