ROLE OF THE SUPERVISOR OF WRAPAROUND CARE COORDINATION

In your vital role as the Supervisor of Wraparound Care Coordination, you will be responsible for assessing referral information, conducting IM+CANS assessments, and ensuring documentation aligns with HFS guidelines. You will work collaboratively with youth and families to create Crisis Prevention and Safety plans, form Child and Family Teams, and facilitate regular meetings. Your responsibilities will include conducting weekly home and community visits, connecting families with appropriate service providers, and coordinating with the youth's MCO Case Manager. Your culturally sensitive and trauma-informed approach will foster effective collaboration with treatment providers, educational institutions, and other stakeholders to meet the holistic needs of the youth. You will maintain timely and professional case records while managing a caseload ratio of 1:10 within your designated service area.

QUALIFICATIONS REQUIRED:
  • Must be at least 21 years of age
  • Bachelor's Degree in counseling, social work, psychology, education, or a related field
  • A minimum of 2 years of supervisory experience
  • Familiarity with community programs, services, evidence-based practices, and treatment modalities for youth and families
  • Ability to thrive in a fast-paced, multidisciplinary team environment
  • Valid driver's license and proof of vehicle insurance
  • Strong organizational and time management skills
  • Excellent interpersonal and written communication abilities
