ROLE OF THE YOUTH AND FAMILY SERVICES CARE COORDINATION LEADER
In the role of Youth and Family Services Care Coordination Leader, you will play a crucial part in the Care Coordination and Support program. Your responsibilities will include promptly evaluating referral information, conducting IM+CANS assessments, and ensuring documentation is updated in accordance with HFS guidelines. You will work collaboratively with youth and families to create Crisis Prevention and Safety plans, establish Child and Family Teams, and facilitate regular meetings. Your role will involve weekly home and community visits, connecting families with appropriate service providers, and coordinating with the youth's MCO Case Manager. Your culturally competent and trauma-informed approach will be essential in collaborating with treatment providers, educational institutions, and other stakeholders to meet the holistic needs of the youth. You will maintain accurate and timely case records while managing a caseload ratio of 1:10 within your designated service area.
QUALIFICATIONS REQUIRED:
- Must be at least 21 years of age
- Bachelor's Degree in counseling, social work, psychology, education, or a related field
- A minimum of 2 years of supervisory experience
- Familiarity with community programs, services, evidence-based practices, and treatment modalities for youth and families
- Ability to thrive in a fast-paced, multidisciplinary team environment
- Valid driver's license and proof of vehicle insurance
- Strong organizational and time management skills
- Excellent interpersonal and written communication abilities
