Join Our Team
CDC has a full-time opportunity for a Health Educator at our Buffalo clinic. This position provides oral health education to children and families in our clinics, and provides outreach education and oral hygiene supplies in a wide variety of community locations. In addition, this role provides care coordination to help families register as new patients and follow up phone calls to schedule dental appointments.

Health Educator Responsibilities:

  • Provide one-on-one education and motivational behavior change coaching to parents, pregnant mothers and children regarding good oral hygiene practices and basic nutrition in our Buffalo clinic.
  • Enroll children 0-12 and pregnant mothers in the Program to Improve Community Oral Health (PICOH) program.
  • Provide phone call and in person follow-up coaching and education to program participants.
  • Send email/mail and text correspondence to families such as appointment reminders and Cavity Free Kids notices.
  • Make phone calls to schedule patients encountered in the clinic or at outreach settings for follow up appointments in the clinic. Provide follow-up support and phone calls for patients with missed appointments.
  • Collaborate with community partners to coordinate and deliver outreach education and awareness raising at a variety of community locations, including schools, health fairs, community events, classroom education/presentations, etc.
  • Initiate and register patient charts and document chart notes in open dental electronic health records.
  • Collect and maintain data on services provided, number of patients served in-clinic and via outreach, and other relevant information for grant and evaluation purposes.

Hours:

  • Monday through Thursday 7:20 am to 4:00pm
  • Friday 7:20 am to 2:00 pm
  • Occasional weekend and evening hours to support community outreach events

Health Educator Desired Qualifications:

  • Interest in oral health care/dental care and community health promotion.
  • Desire to help people learn about and practice good health habits
  • Ability to quickly adapt within a fast-paced work environment
  • Experience working with children and families
  • Experience working with culturally diverse communities
  • 1-2 years experience in education, patient care, child care and/or customer service
  • Dental, health education and/or public health experience
  • Proficiency in a second language desired, such as Spanish
  • Proficient with electronic forms of communication, including e-mail correspondence
