We are seeking a skilled and compassionate Nurse Health Educator to join our team at Endeavor Health Services. As a key member of our interdisciplinary team, you will provide essential health education and support to individuals during critical times, helping them navigate their crises with resilience and informed care.
Key Responsibilities- Health Education and Support: Provide direct medical and educational services to individuals, including group-based health education, health screenings, and health assessments.
- Screenings and Assessments: Conduct screenings and assessments, including vital signs, health literacy education, and other screenings as indicated and assigned.
- Documentation and Reporting: Document encounters with individuals, including presenting concerns, medical history, and recommendations for treatment and linkage.
- Collaboration and Consultation: Work in consultation with counseling staff to triage higher levels of care for individuals experiencing crisis presentations, and consult with staff psychiatrist for psychiatric and medication evaluation when needed.
- Community Linkage and Advocacy: Maintain knowledge of community resources and assist with linkage and advocacy for individuals and their families.
- Education and Experience: Bachelor's degree in nursing, currently licensed as a Registered Nurse by the New York State Department of Education. Additional experience working and/or training in behavioral health is preferred but not required.
- Skills and Competencies: Strong communication and interpersonal skills, ability to work in a fast-paced environment, and commitment to providing high-quality patient care.
