About the Role:
As a Community Health Registered Nurse at Neighborhood Health Center, you will play a vital role in providing high-quality care to diverse individuals and families in our community. We are seeking a skilled and compassionate nurse who is passionate about delivering impactful healthcare while striving for health equity.
Responsibilities:
- Build trusting relationships with patients to facilitate access to healthcare services and meet their health needs.
- Engage patients in health promotion, maintenance, and restoration activities.
- Work in partnership with our multidisciplinary team to advocate for patients and ensure their health needs are met.
- Assist patients and families with accessing, navigating, and connecting with internal providers and services, outside community resources and programs, and other services as needed.
- Revises and reprioritizes plan of patient care, as needed; identifies and addresses any barriers affecting a patient's ability to adhere to the plan.
- Conduct community outreach, home visits, shelter/street outreach to identify people who are experiencing homelessness and other social support needs and educate about health needs.
Requirements:
- Graduated from an accredited two-year nursing school.
- Must possess a current NYS Professional Registered Nurse license.
- Must have current CPR certification.
- The ideal candidate has knowledge of case/care management principles and has the ability to work with a diverse group of patients and understands the impact of social determinants on health.
- Exceptional attention to detail and follow-through.

- Self-starter with the ability to work proactively and take initiative.
