Responsibilities:

  • Build trusting relationships with patients to facilitate access to healthcare services and meet their health needs.
  • Engage patients in health promotion, maintenance, and restoration activities.
  • Work in partnership with our multidisciplinary team to advocate for patients and ensure their health needs are met.
  • Assist patients and families with accessing, navigating, and connecting with internal providers and services, outside community resources and programs, and other services as needed.
  • Revises and reprioritizes plan of patient care, as needed; identifies and addresses any barriers affecting a patient's ability to adhere to the plan.
  • Conduct community outreach, home visits, shelter/street outreach to identify people who are experiencing homelessness and other social support needs and educate about health needs.

Requirements:

  • Graduated from an accredited two-year nursing school.
  • Must possess a current NYS Professional Registered Nurse license.
  • Must have current CPR certification.
  • The ideal candidate has knowledge of case/care management principles and has the ability to work with a diverse group of patients and understands the impact of social determinants on health.
