The Behavioral Health Nurse Care Manager plays a crucial role in delivering comprehensive care and collaborating with various stakeholders to enhance the quality of OASAS substance abuse services. - Conducts thorough nursing evaluations, group sessions, and ongoing health monitoring.
- Manages patient scheduling, tracks attendance, and reviews toxicology results.
- Coordinates with pharmacies and insurance providers to ensure appropriate prescription coverage for patients.
- Facilitates interdisciplinary treatment team meetings to promote collaborative care.
- Maintains and updates a comprehensive list of referrals and resources.
- Provides educational training to non-medical personnel.
- Ensures compliance with OASAS regulations and accurate billing practices.
- Engages with both internal and external healthcare providers for seamless patient care.
- Adheres to OASAS standards and the policies of Evergreen Health.
- Promotes a patient-centered approach in all aspects of care delivery.
Qualified Candidates must possess a valid RN license in New York State, BCLS certification, and a minimum of two years of experience in behavioral health nursing. Familiarity with electronic health record systems is essential. Candidates should demonstrate professionalism, accountability, integrity, and a deep understanding of cultural diversity, LGBTQ issues, harm reduction strategies, substance use disorders, and HIV/AIDS.

Certification Required: RN License in NYS
Experience Required: At least 2 years in behavioral health nursing (experience in a medication-assisted treatment clinic is preferred).
