BronxWorks is hiring Two (2) Case Management Supervisor for Homebase Program


RESPONSIBILITIES

· Review client intakes and determine client eligibility for Homebase services

· Review cases, to ensure that they meet ESG/EAF eligibility requirements

· Ensure that ineligible clients are referred to appropriate alternative services

· Ensure that client records are in compliance with ESG/EAF guidelines

· Review HRA guidelines with staff to ensure that case managers understand ESG/EAF requirements

· Review HRA database frequently to ensure that case management staff is complying with HRA quality assurance requirements

· Conduct weekly supervision with staff and complete supervision forms

· Complete individual monthly reports for line staff

· Review Financial Assistance Requests

· Assess with case managers the risk of clients entering shelter system

· Review and sign all client service plans

· Review and sign documentation in client files, where supervisory signature is required

· Conduct Quality Assurance reviews to ensure cases and case records (paper and electronic) meet ESG/EAF standards

· With Program Director, create disciplinary plan of action for staff not meeting their job requirements


QUALIFICATIONS

· Master’s Degree with proof of completed credits

· Prior supervisory experience working in a social service agency preferred

· Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile and copier equipment

PHYSICAL REQUIREMENTS

· Ability to use a computer for prolonged periods.

· Ability to occasionally lift and/or move up to 10 pounds.

· Ability to stand, walk, or sit for long periods of time.

· Ability to bend and retrieve objects and/or documents.

· Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.

· Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
