Schedule: Monday thru Friday 8:30am-5:00pm


Role Overview:

We are seeking an Intake Coordinator with experience to join our team in {location}. In this role, As an Intake Coordinator, you will be responsible for the initial evaluation of clients, gathering comprehensive information about their medical history, current health status, and specific needs. This information is crucial in developing personalized care plans aimed at improving clients' quality of life.


Key Responsibilities:

  • Coordinate with healthcare providers, patients, and their families to gather necessary information for intake procedures.
  • Input and maintain accurate patient records in the company's database system.
  • Schedule appointments and coordinate with caregivers to ensure timely delivery of services.
  • Provide excellent customer service by addressing inquiries, resolving issues, and maintaining positive relationships with clients.
  • Assist with insurance verification and documentation processes. Collaborate with the billing department to ensure accurate billing and payment processing.
  • Support administrative tasks such as filing, faxing, and emailing as needed.
  • Maintain confidentiality of sensitive patient information in compliance with HIPAA regulations.
