Full Time Housing Case Manager

General Description: The Housing Case Manager will work with 18-24 young adults in the Thrive Housing Program. Duties will include tasks to support and partner with CoC and/or YHDP case managers in support of client service plans.

Essential Responsibilities - Housing Exit Planning & Coordination
  • Involvement in post-housing program planning for youth approaching the last 60 days of their housing program (ensuring notice has been given to apartment or lease has been renewed
  • Assist and support housing search & placement, assistance with move-in, coordinating with case manager regarding move-in to ensure a smooth transition
  • Conduct exit survey with youth & uploading to HMIS, entering final move-out plan or lease renewal plan into HMIS, entering final income information into HMIS, explaining aftercare process, ensuring youth understanding of the ability to contact Thrive at any time for resources to help direct them toward diversion/not returning to homelessness)
  • Coordinate with and inform case managers of any additional concerns as housing program exit approaches (ex: youth needing group-home placement, youth with no employment & lacking a plan - establishing resources with the youth ahead of time)
  • Conduct check-ins as-needed and as-requested by case manager with youth in person or by phone (ex: checking in with a youth to provide time for case manager to attend court, or checking in if CM is on leave)
  • Provide aftercare services with clients (phone / email / text; in-person visits would only be conducted on a case-by-case basis as needed) providing any resources youth may need.
  • Update all relevant HMIS data and documents, including a housing-exit summary case note for the youth
  • Submit referral forms for benefits or education/employment assistance as needed
  • Assist with acquiring identification documents
  • Research, locate and obtain resources as needed for clients
  • Utilize on campus work space when not in youth meetings
  • Ensures the timely completion of required documentation, and ensures required information is entered into the appropriate web-based system and HMIS as required
  • Responsible for adhering to all policy and procedures as outlined in the Employee Handbook and other departmental procedure manuals

Other Duties:
  • Able to have daily use of a vehicle without prior notice
  • Maintain current registration and automobile liability insurance
  • Be available and willing to transport clients to various appointments (documented mileage reimbursed)
  • Have a clear criminal background check
  • Must have use of a cell phone
  • At times, participate in activities outside of regular responsibilities
Education
Required: Bachelors Degree in Social Work / Psychology or a related field

Experience
Required:
  • Two years of experience in case management services within community programs serving youth, at-risk young adults, and/or homeless individuals
  • Knowledge of community resources
  • Knowledge of trauma-informed care
Preferred:
  • CPR & BLS Certification
  • Knowledge of HMIS
  • Fluency in English/Spanish preferred
