Summary/Objective:

The Foster Care Healthcare Coordinator is responsible for ensuring the provision of health care services the highest quality, accessible and properly utilized by youth in CHJC’s Foster Care Program. The Healthcare Coordinator collaborates with the Foster Care Department and the Community Clinic of Jefferson County to ensure necessary assessments are completed, medical necessity is documented for billing purposes, treatment plans align with current needs, and the youth receives the necessary comprehensive, preventive, and emergency medical/dental/behavioral health care as needed. The Healthcare Coordinator participates as a member of the CHJC Treatment Team, prepares necessary reports, and educates youth and their families.


Essential Functions

1. For each new admission, completes 24-hour initial medical screen, documents in the electronic medical record, makes recommendations, and shares findings to all necessary parties

2. For each youth, completes the required HIV risk assessment/screening and provides outcome of assessment to the assigned case planner

3. Obtains a complete medical history, obtains records from previous providers and meets with birth parents/previous caretakers

4. Maintains documentation of the youth’s medical history and treatment in CHJC’s electronic medical record

5. As requested, inputs necessary documentation into CONNECTIONS

6. Regarding medical procedures, medication, and disease/injury, provides education/counseling to the youth, foster parents and/or birth parents

7. Ensures each youth is scheduled for necessary preventive appointments and as needed, assists the family in establishing new providers

8. To ensure the needs of the youth are being met, maintains contact with community-based providers

9. Assists with referrals for specialty healthcare, follows-up regarding recommendation


Special Requirements:

Work Environment: Office environment will require occasional travel to recruitment events, local schools, conferences and/or meetings and moderate travel within the community to include off-site/in-home visits. May have contact with service recipients that are agitated and/or are in crisis. Some risk involved working with service recipients with mental illness. Works in an office setting with a controlled temperature environment. Occasional exposure to inclement weather conditions may occur depending upon assignments. Exposure to blood and/or body fluids.

Equipment: This position is required to maintain a working knowledge of related office equipment including personal computers and printers, audio-visual equipment, telephone systems, copiers, fax machines, etc.

Physical Demands: Must be able to sit, stand, walk, lift, carry, push/pull, climb, bend, and stoop. Must be able to perform fine motor skills, read, and type. Must be able to sit for long periods of time. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.


Position Type/Expected Hours of Work:

Full-time and Part-Time, traditional hours required along with occasionally some non-traditional hours. This position is required to work on-site.


Travel:

Travel is required throughout service area (Jefferson, Lewis, Oswego, and St. Lawerence County).


Required Education and Experience:

New York State Department of Education Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN).

Additional Eligibility Qualifications:

Must maintain privacy of confidential information. Successful completion of routine pre-employment requirements as mandated by the New York State Department of Health, Office of Mental Health and Office of Children and Family Services along with ongoing clearances and exclusion checks. These requirements are to include employment health requirements, reference checks, criminal background checks/fingerprinting, State Central Registry clearance and your eligibility to work in the United States. A valid NYS driver’s license required. Corporate Compliance, HIPAA Privacy Security, and Workplace Harassment and Discrimination Training and Nonviolent Crisis Intervention, First Aid and CPR will be required following date of hire.
