Job Description

Davis Behavioral Health is seeking a full-time Case Manager to join the Children, Youth, and Families Mental Health outpatient team. The Children, Youth & Families Case Manager is a bachelor's level mental health professional who provides targeted case management, behavior management, and skills development services to children and their families. The case manager assists clients in effectively utilizing available community resources. Case management can be provided in the home, office, school, and community settings.

Primary Job Duties

The Children, Youth, and Families Case Manager is responsible for the following job duties:

  • Conduct in home needs and strengths assessments.

  • Develop individualized service plans consisting of mutually agreed upon goals and objectives and update as required.

  • Link families with systems of care that provide them with resources, services, and opportunities to improve their quality of life.

  • Advocate for individuals and families who need assistance while encountering barriers in connecting with systems of care.

  • Provide individual, family, and group skills using evidence-based interventions to decrease symptoms and improve functioning. This is done in conjunction with the assigned mental health therapist or prescriber.

  • Assist families in gathering natural supports and resources for becoming independent of agencies.

  • Maintain clinical documentation as required by agency standards.

  • Coordinate with other staff and community agencies as needed to provide effective care.

  • Participate in weekly staff meetings and regularly scheduled supervision.

  • Must meet agency productivity standard.

  • Other duties as assigned and needed.

Compensation

The hourly rate for this position is outlined below

  • $25.50 per hour + for candidates licensed as a Social Service Worker (SSW) in the State of Utah.
    • 1% per year for each year of licensure.
  • $21.50 per hour for candidates with a bachelor's degree in a social science field, but no SSW license.
    • 1% per year for each year of licensure between 0 and 10 years.
  • $18.25 per hour for candidates without a bachelor's degree.
    • 1% per year for each year of licensure between 0 and 10 years.

Schedule

This is a full-time position and is normally assigned to weekdays. This schedule may require three evenings per week. DBH reserves the right to require overtime, weekend work, and/or assignment to evening or night shifts, depending upon operational need, some in and out of state travel may be required.

The proposed schedule for this position is below:

  • Monday 11:00 am to 7:00pm
  • Tuesday 9:00 am to 5:00 pm
  • Wednesday 11:00 am to 7:00 pm
  • Thursday 11:00 am to 7:00 pm
  • Friday 9:00 am to 5:00 pm

Required Experience

Candidates licensed as a Social Service Worker (SSW) are strongly preferred. Candidates who are not currently licensed but are actively pursuing SSW licensure or those that do not have a SSW license, but are eligible for Targeted Case Management Certification may also be considered.

Skills/Abilities/Qualifications

The following skills, abilities, and qualifications are essential for this position:

  • Knowledge of community resources.

  • Demonstrated ability to motivate and engage youth and their families.

  • Knowledge of the techniques and principles of child and family development and family systems theory.

  • General understanding of mental health and substance use disorders.

  • Good organizational and practice management skills.

  • Ability to work independently and effectively.

  • Ability to establish and maintain effective working relationships with other staff and community agencies (such as DCFS, juvenile courts, schools, etc.).

  • Flexibility in days and hours available for scheduled work, including evenings.

  • Davis Behavioral Health utilizes an electronic medical record, which requires proficiency in computer-related skills.

Meeting monthly face-to-face service expectations and productivity standards is also required.

The selected candidate must successfully pass a pre-employment criminal background check and drug screening. DBH will not hire individuals currently using illegal drugs or abusing alcohol; therefore, making employment at DBH conditional upon successfully passing pre-employment drug screening by providing a valid sample. Candidates who are offered employment are required to report for a drug screen at an Intermountain WorkMed location within 24 hours of being requested to do so by the DBH Human Resources Department.

Targeted Case Management Certification

Candidates who do not hold a professional license must complete certification as a Targeted Case Manager (TCM) within the first 30 days of employment.

Qualifications to become a certified Targeted Case Manager through the State of Utah Division of Substance Abuse and Mental Health include:

  • 400 hours of work, volunteer, and/or education experience in human services or a related field

  • Required 40-hour case management practicum and examination provided by DBH during the TCM certification class.

Preference will also be given to candidates who are bilingual English/Spanish.

The successful candidate must also have a clean driving record to operate agency vehicles and/or transport clients, when necessary. The ideal candidate will have experience working with clients that have sever mental illness, are hard working, can work both independently and successfully within a small outreach team.

DBH Criminal Background Check & Drug Screening Disclaimer

Davis Behavioral Health is a drug-free workplace and requires all employees to successfully pass a pre-employment drug screening.
