What will you be doing:

We have an opening for a Hospice Triage LPN or RN. This position is responsible for managing all aspects of patient calls ensuring that all customers are treated with the highest level of customer service. Provides support to Hospice Clinical Manager and clinical field staff. This is a weekend only position, Sat/Sun, 10am to 8pm.

  • Facilitate the collection of information required in triaging patient inquiries
  • Provides support to the clinical staff in the field by coordinating phone calls to physicians, patients, and families regarding medication, treatments and management of symptoms
  • Works closely with Hospice Liaison Team in the coordination of referrals
  • Ensures that all referrals being processed for admission comply with the Medicare Conditions of Participation
  • Demonstrates exceptional customer service to patients, families, referral sources, co-workers and other customers
  • Assures compliance with all federal and state laws as well as agency compliance policy in the prevention of fraud and abuse


What we are looking for in you:

  • LPN or RN license required (in good standing)
  • 2+ years in an office setting, 1+ year in home health preferred
  • Hospice Experience a plus
  • Strong organizational skills
  • Ability to multi-task in a fast paced environment
  • Strong interpersonal skills
  • Able to communicate effectively and courteously, both verbal and written
  • Able to troubleshoot, problem-solve quickly and efficiently
  • Familiar with Electronic Medical Record
  • Ability to work in office: 2 days per week, Fridays and Saturdays, 10am-8pm (20 hrs/wk)
