Position Summary

The Patient Care Coordinator is responsible for the development, coordination, and maintenance of the care continuum for all patients at the ASC with special emphasis on Total Joint and Spine procedures. The Coordinator works with the multidisciplinary team to coordinate, integrate, and evaluate patient care protocols to enhance outcomes. The Coordinator serves as a liaison and resource to the patient and the multidisciplinary team including allied health professionals. The Coordinator acts as a patient advocate and educator to plan, organize, coordinate, and monitor the provision of quality and efficient healthcare services and resources and ensure that the patient has the knowledge to care for his/her condition and participate in their plan of care.

Duties and Responsibilities

  • Functions as a core leader for the total joint and spine specialty programs.
  • Facilitates the development of the outpatient total joint and spine program to support the growth of total joint and spine surgery in the outpatient setting.
  • Provides guidance and oversight for the admitting and recovery room staff to ensure that a patient with extenuating circumstances is provided additional support during their stay and postoperative course of recovery.
  • Makes the initial contact with the patient to introduce them to the Center’s team.
  • Conducts an extensive health assessment and screening and notifies the surgeon and anesthesiologist of any concerns regarding surgical and/or recovery risks.
  • Provides education to the patient regarding presurgical equipment and home, day of surgery routines, and recovery at home routines.
  • Coordinates Home Care and Physical Therapy as indicated.
  • Completes patient follow-up phone calls and data collection post discharge at specific intervals per program guidelines.
  • Works closely with all physicians to ensure protocols are followed and education materials are current.
  • Trains and supports all necessary teammates to participate in total joint and spine cases.
  • Ensures provision of patient care in an organized and timely manner.
  • Participates in Quality Improvement/Patient Safety meetings and projects.
  • Works closely with the multidisciplinary team to collect data for tracking and trending quality outcomes.
  • Reports Incident and Adverse Incidents per Center policy.
  • Participates in appropriate clinical meetings to ensure continuum of care.
  • Performs all other duties as assigned.

Knowledge, Skills, and Abilities

  • Current certification in Basic Cardiac Life Support, Advanced Cardiac Life Support and Pediatric Life Support required (able to obtain on site within 6 months if needed)
  • Orthopedic Nurse Certification required within 6 months
  • Maintain membership and activity within a relevant professional organization. (for example: AORN, NAON, ASPAN)
  • Must be able to set priorities and provide directions to others that are clear, concise, and promote efficiency.
  • Regularly access internal and external resources to maintain professional knowledge base.
  • Annual completion of specified competencies and in-services.
  • Respects the patient’s right to privacy, dignity and confidentiality and will provide a safe a supportive patient atmosphere.
  • Maintains attendance according to facility policy.
  • Ability to work flexible hours to meet the needs of the program and the patients.

Education, Experience, Licenses, and Certifications

  • Licensed as a Registered Nurse (RN) in the State of Michigan
  • Bachelor’s in Nursing or equivalent preferred
  • Previous work experience as a Registered Nurse on an Orthopedic floor, or in an Ambulatory Surgery Center, or in the Operating room circulating orthopedic procedures, or the Recovery Room caring for postoperative orthopedic patients- Required
  • Quality Assurance/Quality Improvement experience: Preferred
  • MARCQI and/or MSSIC experience: Preferred
